Adding & Previewing Emergency Contact on a Form

Emergency contact details can be collected directly through BETA Forms and previewed in a profile without generating a PDF. This helps ensure important contact information is captured accurately during sign-up.

Emergency Contacts are not collected by default, they need to be added as a section in the form. This allow you to control in which, and when in the form this information is collected.

› Adding an Emergency Contact collection to a Form

Emergency contact details are added to a form as standard Input Fields, in the Edit Form Content section when editing a form. This allows users to enter the information during form signing process.

We recommend adding the following INPUT FIELDS:
» Emergency Contact Name
» Emergency Contact Phone Number

as two separate fields to keep the data clear, and consistent.

This information is saved to the form, and pre-viewable on the profile once the form is completed.

› Previewing a Form, including Emergency Contact

Previewing a form from the Profile shows the exact values entered by the person who signed it, including what they filled in or selected. This allows you to quickly review submitted information without generating or opening a PDF.

Attention! Migrated Forms? Migrated Emergency contact fields are viewed differently.

Migrated forms Emergency Contact details are stored as Comments (Business or above) or in the Tags (Community and below) section of a profile.
